I confirm that the child(ren) listed is/are under 16 years old but above 3 years of age and that the child is over 1 meter tall.

I wish the child(ren) to participate in indoor trampoline activities including, but not limited to trampolining, trampoline park access, trampoline dodgeball, trampoline basketball, foam pit, rope challenge, airbag jump, café access, (collectively hereinafter called “the Activity”) organized by Gravity Indoor Trampoline Parks trading as Gravity.

I declare that I am the parent or legal guardian of the child(ren) or I have authority from the child(ren)’s parent or legal guardian to sign this Waiver Form.

I agree that the child(ren) will purchase and wear the Gravity rubber grip socks in accordance with the specific safety instructions that will be given before undertaking the activity.

I agree that the child(ren) will wear any safety equipment provided by Gravity in accordance with the safety instructions and that he/she will follow the directives of Gravity, its management and staff at all times.

I confirm that the child(ren) is in good physical condition and has no medical impairment that might prevent him/her from the intended use of Gravity.

I acknowledge that Gravity did not give me medical advice relating to the child(ren)’s physical condition and ability to use the facilities.

I acknowledge that the Activity is potentially dangerous and that by participating in the Activity the child(ren) will be exposed to the possibility of personal injury.

I hereby indemnify Gravity against all claims made by any other person against Gravity in respect of any injury, loss or damage arising out of or in connection with the failure to comply with the safety instructions and /or directions of Gravity, its management or staff.
